  Advance Directives for End of Life Care Bookmark and Share

    Advance directives are written instructions regarding you or a loved one's preferences for medical care at the end of life. Ideally, you should discuss end of life preferences with your family or care provider while one is still able to communicate their wishes. Later, make sure copies of advance directives are included in one's medical charts. This is especially important if you or a loved one moves to a nursing home or other facility for long term care. The advance directives will help the staff know what level of treatment is to be carried out in medical emergencies.
